Enrollment Forms

Each year in February, new and returning families enroll or re-enroll their child(ren) for the upcoming school year.  For new families, Enrollment Agreements will be sent following the admissions’ process.

For the 2019-2020 school year returning families, must complete, sign and turn in Enrollment Agreements with Enrollment Fees by February 23, 2019. Agreements are received by new families on a rolling basis, space permitted.

For New Families, Enrollment Forms are completed after Application for Admission and Admission Process is followed.  Any questions or request for forms can be obtained by emailing Shari Herszage at sherszage@torahacademy.org or downloading a blank form here (AVAILALBE BY JANUARY 15).

FACTS TUITION MANAGEMENT

Tuition can be paid in full or by installments.  All families must enroll in our Tuition Management program through FACTS.  Tuition Manangement Agreement can completed at https://online.factsmgt.com/signin/3WQGJ  CTA school code is 16016.  Please note: There is a one-time fee of $50 to FACTS which will be assessed on the first tuition payment.  Tuition payments for 2019-2020 will begin in September 2019 if you are on a payment plan. If you pay in full up front the due date is August 1, 2019.

Financial Assistance

Columbus Torah Academy awards financial aid to families based solely on financial need.  Families requesting any level of financial assistance must apply online through FACTS. Financial aid forms should be started by March 15, 2019.  All deadlines must be adhered to.

CTA may assess a $500.00 late fee for all FACTS applications filed after March 15, 2019.

To apply for Financial Assistance, you must upload your 2018 W-2’s and 2018 1099’s and, when available, 2018 tax return for analysis.  The FACTS results will be evaluated by the CTA Scholarship Committee. In the event that you are unable to provide your 2018 tax return due to extension of your tax return submission date, then you must submit your 2017 tax return along with a copy of your 2018 extension.  You will be required to submit your 2018 tax return by October 15, 2019.

CTA is dedicated to assisting families in financial need.   CTA uses FACTS as a third party to review all applications submitted by families to provide guidance to the Scholarship Committee in providing this assistance.  CTA must rely on the submitted documents to be truthful and correct. In order to maintain compliance the Business Office reserves the right to audit any application to verify accuracy.  This may require an applicant to sign an IRS From 4506 requesting a certified copy of Form 1040 for the respective year.

If a FACTS application is found to be inaccurate, the forms and documents are to be resubmitted and tuition will be re-evaluated accordingly.  The scholarship committee, at their discretion, can assess penalty of up to $1,000 and/or decide to withhold any or all financial assistance for that year.  In addition, all financial data submitted for prior years could be subject to review. If errors or omissions are found for any of those prior years, tuition will be re-evaluated and the family can be requested to pay tuition for monies owed from those prior years.   A payment plan for any prior year tuition owed will be established with the finance office. Any costs associated with the audit and or review may be billed to the respective family.
